Authorization for the processing of personal data

I declare that I have been informed:

1. That INTEGRAL DE CARGA, whose trade name is “CARGRANEL S.A.”, identified with NIT 800038129-8, domiciled at Calle 77 sur No. 47B-46, Sabaneta, Colombia, will act as the controller of my personal data and has made available the phone line 2883878, Monday to Friday from 8:00 a.m. to 5:30 p.m., and the website www.cargranel.com, for requests related to the processing of my personal data and the exercise of the rights mentioned in this authorization.

2. My data will be processed for administrative, commercial, advertising and contact purposes with the data subjects.

3. My rights as data subject are those provided in the Constitution and the law, especially the right to know, update, rectify and delete my personal information, and the right to revoke the consent given for the processing of personal data. I can exercise them through the channels provided by CARGRANEL and in accordance with CARGRANEL’s information processing policy, available at www.cargranel.com.

4. It is voluntary to answer questions that may be asked about sensitive data or data of minors, and the latter will be processed respecting their fundamental rights and best interests. Sensitive data are those that affect the privacy of the data subject or whose misuse may cause discrimination, for example political orientation, religious or philosophical convictions, human-rights data, and data related to health, sexual life and biometric data.

In view of the above, I voluntarily, previously, explicitly, in an informed and unequivocal manner authorize CARGRANEL S.A. to process my personal data, with the main purpose of carrying out the corporate purpose and the contractual relationship, execution and provision of the transport service of the controller, as well as contact by telephone, electronic means (SMS, chat, email and other electronic means), physical and/or personal means.

CARGRANEL S.A. may use my information for the following purposes:

1. The development of the corporate purpose of CARGRANEL S.A. and of the commercial, employment or other existing relationship with the data subject.

2. The development of all activities established in contracts or agreements.

3. Carry out the steps required for the pre-contractual, contractual and post-contractual stages with CARGRANEL S.A., regarding any of the services it offers, whether or not acquired, and to comply with the law and orders of judicial or administrative authorities.

4. Comply with obligations undertaken with our clients, contractors, suppliers and employees.

5. Carry out internal studies on client needs.

6. Properly handle petitions, requests and complaints.

7. Provide contact information and relevant documents required by clients, suppliers or supervisory bodies that have any contractual link with CARGRANEL S.A.

8. Access and consult my personal data held in databases or files of any private or public entity, including ministries, administrative departments, superintendencies, DIAN, the Prosecutor’s Office, courts and high courts.

9. Create databases for the purposes described in this authorization.

I give my consent to CARGRANEL S.A. to process my personal information in accordance with the personal data processing policy, and I undertake to read the privacy notice and the policy available at www.cargranel.com. I authorize CARGRANEL S.A. to modify or update its content in order to address legislative reforms, internal policies or new requirements for the provision or offering of services or products, with prior notice through the website and/or email.

The information in the form of which this authorization is part has been provided voluntarily and is true. I acknowledge that the data supplied in the request are accurate and that no information has been omitted or altered. I have been informed that falsehood or omission of any data will make it impossible to provide the requested services correctly.

This authorization remains in force until it is revoked, and it may be revoked in the cases provided by law, provided there is no relationship of any kind with CARGRANEL S.A.
